• Ministry Application
  
  • If you're interested in volunteering in any ministry at 'the shoreline', you MUST have a volunteer application on file with us.
  • Supplement for female volunteers to read
• Adult Medical Release Form or Minor Medical Release Form
  
  • Every person participating in a church trip of any sort must have a new form on file each year.
• Meals Request Form
  • Please use this form to submit a meals request for someone in need of meals.  You may submit the form to Kristy Johnson at kristylynnh@yahoo.com
• Check Request Form
  • If you are a volunteer and have made a purchase for the church, please fill out this form and either fax or mail it in to us with the receipt(s).  We will mail you a reimbursement check within a week.
• Scholarship Application 
  • If you would like to go on a church trip, but need financial assistance, then please fill out this form and turn it in to the trip leader or to the church office.  Someone will get back to you within a few days of turning it in.
